*A Note about residency verification and birth certificates: When you register there is a document upload portal for pertinent documents to verify residency and age. PLEASE upload these documents, if possible, for the 2024 season. If you have previously uploaded documents, you will need to upload them again due to our server automatically deleting them after 6 months (the exception is for birth certificates, once those are uploaded, they remain in the player profile. Residency must be verified each year). If your child decides to play all-stars, we will need to see an ORIGINAL birth certificate at that time.

If you are using this form, please print it out, have your school administrator sign it, then upload to the document portal.
